Small-Bite Closure Reduces Long-Term Hernia Risk

In a multicenter randomized clinical trial involving 559 patients from 10 centers in the Netherlands, small-bite fascial closure significantly reduced the long-term incidence of incisional hernia compared to large-bite closure for elective midline laparotomy, with findings published in JAMA Surgery. Over a median follow-up of eight years, the cumulative hernia incidence was 34% in the small-bite group versus 49% in the large-bite group. Furthermore, the size and progression rate of hernias developed through small-bite closure were more favorable. Despite consistent supportive evidence, adoption of this closure technique in practice remains low, highlighting the need for improved education and standardization among surgeons.
